Course Description

This request should be completed by a student wanting to obtain a reissue of their White Card. This only applies for White Cards issued by QLD and WA. To complete this form you must satisfy the below criteria: 1. The original card was issued by Allens Training Pty Ltd RTO 90909 OR 2. The original card was issued by another RTO that you are unable to contact but you have a copy of your original statement of attainment. AND 3. You have worked within the construction industry consecutively for the last two years. If you do not meet the above criteria please do not proceed as we will not be able to re-issue you a White Card.
